Influencing transfusion practice utilising account for patient data

Background
It was recognised in January 2016 that NHS Grampian’s cross - match to transfusion ratio (C:T) was one of the highest in Scotland.
A collaborative improvement project between SNBTS and NHS Grampian clinicians was developed to reduce the high C:T ratio and translate this into a review of the Maximum Blood Ordering Schedule(MSBOS). The reviewed MSBOS would ensure that blood was used more effectively and reduce unnecessary cross-matching.
Method:
Utilising account for patient data, we presented the high C:T ratios to the Hospital Transfusion Committee. We targeted wards which had the highest usage and C:T ratios, then engaged with surgeons and their teams at departmental meetings. An action plan was developed for each clinical area with ongoing monitoring of adherence to the agreed schedule.
Results:
Continued tracking, trending and engagement with clinical teams has seen a reduction of the C:T ratio in Grampian, from 2.2 prior to interventions to 1.8 in March 2017.
Conclusion:
This collaborative approach has resulted in a more effective use of the donors’ ‘gift’ and saved labour and consumables within blood bank.
Presenting Account for Patient data to clinicians can be a powerful tool to effect positive changes in practice.
In order to embed this change, key interventions need to continue:
- use data to track and trend progress
- continue to engage with clinicians to review practice
- celebrate successes
- promote the agreed MSBOS to ensure standardised practice particularly amongst rotational clinical staff
